Welcome to Family Eye Care Center, practice of Dr. Craig Hyre. We are delighted you have chosen us as your eye care provider. We are both a vision and medical optometric physician practice utilizing state-of-the art technology for the diagnoses and management of vision and medical ocular conditions. We specialize in diagnosis and management of medical eye disease, well vision and contact lens exams!

Please print the patient information and HIPAA explanation forms so that you may complete at your convenience. This will save time for you when you arrive for your appointment. Plan to arrive fifteen (15) minutes early to allow for completion of your registration.

When you come in for your initial appointment, please bring with you the following:

1. 1)  Completed Welcome and HIPAA Forms.
2. 2)  All Medical and Vision Insurance Cards.
3. 3)  A List of your Current Medication(s), Supplement(s), and Medication Allergies.
4. 4)  Your Current Spectacles (Glasses), and/or Current Contact Lenses (box or prescription)
5. 5)  A Current Photo ID, and
6. 6)  A Valid Form of Payment for Private Pay or Non-Covered Insurance Balances.

We look forward to meeting you. Should you arrive more than fifteen (15) minutes late, you may be asked to reschedule your appointment.
